New installations and replacements on covers are required to follow this rule:

Receptacles mounted to and supported by a cover shall be held rigidly against the cover by more than one screw or shall be a device assembly or box cover listed and identified for securing by a single screw.

The intent of the requirement is to have this "2-Gang Square Duplex Receptacle Cover" installed because as the image above shows the 6/32 is missing, or ready to fall out. People pay no attention to this and still try to plug in a cord cap.
